How do I set up the left border?

Set up the page as a two-cell table with the border tile as the background in the left-hand cell. See below.

Insert the border tile into the background attribute of the TD tag. Use a vspacer.gif as cell content, width set just under the width of the tile, height set to 1.

Insert the body of the page here. The TABLE and /TABLE tags should go immediately between the BODY and /BODY tags of the page. Leave BORDER and CELLSPACING attributes at 0 in the TABLE tag. They are set at 7 and 2, respectively, for demonstration purposes only.

What about the top-border?

Same idea as the left-border, only you'll put the first cell on top, rather than beside, the body of the page.

Set the height of the vspacer.gif a pixel or two under the height of the border tile, which is still used as the background in this set-up, and the width of the vspacer.gif at 1.

Remember to start a new TR with this layout. Border and cellspacing still left at 0.

Help! I need a button that says [insert title here] but the set I like doesn't have it!

Save the blank button. Open up your graphics program. Open up the blank button file in your graphics program. Insert desired title in the font listed in the set, or a close font thereto, in as close a color and size to the rest of the set as possible. Save file under a different name (to keep a copy of the blank button intact). Repeat process for as many buttons as you need. The nameplate goes the same way.

B-b-b-b-but, my graphics program is really crappy and can't edit .gifs!

What's that little "Graphics by Red Monster" button for? I know you made the graphics, why bother making a button for it?!
